1. Complete and record a functional health assessment of an individual (see criteria).Conduct a thorough assessment of psycho-social-cultural concepts and identification of risk behaviors.Include an assessment of the individual’s beliefs about health.Criteria for selection of primary individual include:

  • At least 55 years or older
  • USE INITIALS ONLY to preserve anonymity of patient/family (no identifying information)
  • Someone who agrees to be interviewed and work with you (in person) on this project

2. Analyze the health assessment data and develop a plan of action. Talk to your “patient” about ideas for the health plan.Apply theoretical concepts to the plan. The plan of action needs to be focused on health promotion, modification of risk factors and/or disease prevention using evidence from at least 3 sources of professional health care literature (professional, refereed articles).

3. Discuss principles of change needed to implement the plan. You may also include the patient’s family in the assessment and plan.

4. Identify at least two (2) community resources available for your individual or family. Agencies could be local, regional, or part of a state or national system. They could be public or private.

5. Research the two (2) community agencies to obtain information. This can be a physical visit or accomplished searching the web. Information to include in your project includes: a) the name of the agency, b) mission and purpose, c) goals and objectives of the agency, d) source of funding, e) who the agency is responsible to, f) management and administration, g) staffing (types of personnel, number of each type), h) what type of care/service is provided by this resource and (i) how does it relate to health promotion for your individual or family?

6.Present your results in a concise manner by developing an online power point presentation. Post your power point (5-11 slides). The presentation should be a clear description of the steps of the project and the community resources you investigated. Project needs to be organized in a way that communicates your findings in a logical manner.

8.Include a slide that lists the references used in preparing your presentation and cite information appropriately using APA format for citing references. This is in addition to the required 5-11 slides.
